1st Grade: Superficial Ulcers

In this stage, the patient develops open wounds such as blisters, corns, or calluses caused by skin damage. These superficial ulcers may be the result of friction, burns, or frostbite, but they do not yet extend into the deeper tissue layers. While the condition may not seem severe, it is crucial to prevent further progression with proper wound care.

2nd Grade: Deep Tissue Involvement

In the second grade of diabetic foot, the infection penetrates deeper into the muscle tissues, often resulting in mild cellulitis and multiple abscesses. The infection spreads through the muscle planes, potentially causing perforating ulcers or gangrene. There may also be purulent discharge, and focal dry gangrene may occur in the foot or toes. At this stage, however, the tendons and ligaments are not yet compromised.

3rd Grade: Tendon and Ligament Involvement

By the third grade, the infection has reached the tendons and ligaments. Cellulitis often coalesces to form large abscesses filled with pus and necrotic tissue. While dry gangrene may develop in some toes or areas of the foot, bone destruction is not yet evident.

4th Grade: Bone Infection

In this more advanced stage, the infection severely affects the bones, resulting in osteomyelitis. Bone and joint structures may begin to deteriorate, and in some cases, pseudoarthrosis or Charcot joints develop. Some toes or parts of the foot may be affected by wet or dry gangrene, increasing the risk of amputation.

5th Grade: Extensive Gangrene

At this final stage, most or all of the foot is infected or ischemic, resulting in widespread wet or dry gangrene. The skin may turn black, indicating severe tissue necrosis. In many cases, the infection extends to the ankle joint and lower leg, requiring high-level surgical amputation to prevent the infection from spreading further.
